Hilbrand Jacobs is project manager for guiding transformation in health and welfare. Among other things, he is carrying out a major assignment for the municipality of Deventer to bring about movement there based on the vision and method of working from Sickness and Care to Health and Behavior. “The change process is based in part on Pim Valentine’s ‘Rainbow Model of Integrated Care.’ Because I was eager to go deeper, I took the Care Network Masterclass.”
Incredibly good
“I found the master class incredibly good. Pim took us through the thinking behind the Rainbow Model, using the scientific knowledge he has accumulated. The model describes the key factors for success for change processes aimed at integrated care. I recognize these factors from the practice of guiding such a process. Such a process can only succeed if you approach it as a value-driven development process.”
Systematic thinking
“Then we made the model concrete and started applying it in a business case. I found the systematic way of thinking strong. How do you keep the needs of the residents leading? What is the role of healthcare? How do you know if you are achieving your goals? What interests play a role? And how do you make a business case with sustainable funding? You can talk about collaboration, but if you don’t attach a common interest and shared risk to it, it will remain non-committal and you will never achieve sustainable change.”
Bearing financial risks together
“The most important thing is that you explicitly follow and reason with the resident. Then it turns out that you do have to cooperate across domains. Ideal is a cooperation in which you also bear the financial risks together. A challenge because of the divisions between the forms of financing. So you need the support of the municipality and the insurer. That is a lengthy process, but in my vision you will reach a point where you will indeed bear those risks together.”
